About the Role:
Job Summary
The RN Transfer Specialist, coordinates in-bound transfers/direct admits from primary care providers offices, free standing emergency clinics (Urgent Care) and patients from regional community hospitals who require a higher level of care according to established policies. The RN Transfer Specialist facilitates activation of the STEMI protocol. Is a professional member of VOICE professional nursing shared governance providing feedback on issues being addressed at meetings. Participates as a member on VOICE councils and Nursing Quality Teams as appropriate. Actively participates on Unit Council helping with plans to improve NDNQI nursing sensitive indicators, RN Satisfaction, Employee Engagement, Culture of Safety, and Patient Experience data. Works together with other care team members to recruit and retain an excellent nursing care team.
Minimum Job Qualifications
Licensure or other certifications: Licensed to practice as an RN in Georgia.
Educational Requirements: Associates Degree in Nursing.
Minimum Experience: Three (3) to five (5) years of experience in direct patient care and/or case management.

Preferred Licensure or other certifications: Professional certification in specialty as eligible.
Preferred Educational Requirements: Bachelors Degree in Nursing.

Essential Tasks and Responsibilities
Provides utilization and resource management by placing appropriate initial patient status on all patients bedded, to include direct admits or as a result of a transfer from another acute care facility.
Coordinates appropriate patient entry into NGMC's acute beds, utilizing clinical criteria to assess medical appropriateness and patient status.
Utilizes second level review process per established guidelines and provides appropriate documentation of referral and outcome.
Identifies and reports clinical transfer issues, utilizing knowledge of EMTALA laws, medical necessity, and reimbursement to manager or director.
Adheres to all regulatory and DNV requirements. Provides consistently legible documentation and/or signatures. Enters orders received appropriately, including date and time.
Knowledgeable of third party/governmental payer regulatory requirements and adheres to appropriate processes/completes paperwork as required.
Maintains confidentiality and respects patient's privacy. Performs in a manner that respects HIPAA laws.
Promotes compliance with external regulatory and health plan standards and requirements to facilitate accurate reimbursement.
Assists other team members to accomplish their job duties. Must demonstrate data entry/retrieval and other computer experience as necessary, and accurately completes all documentation required for all transactions.
Actively supports a customer service oriented environment to continually enhance customer satisfaction.
Communicates directly with physicians and referring facilities to ensure collaborative practice.
Contributes to the productive and effective operation of the PLACe department, including assist in bed placement procedures PRN. Reports problems as opportunities for improvement to management.
Participates in training and orientation of staff members to maintain staff competency and ensure knowledge is updated as more services/duties are added to the department. Orients and educates new employees to the department as needed.
Attends/completes mandatory education courses as required by the hospital and/or manager.
Assists Access Center LPNs and non-clinical staff, as needed, to determine appropriate patient placement using clinical knowledge.
