Job Description

GENERAL PURPOSE: 



The LVN performs selected nurse care under the direction of the registered nurse for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient/family and referring agency.   Ensure staff and other support resources are appropriately utilized and a high level of interdisciplinary team work is maintained through delegation of duties.  



QUALIFICATIONS: 





  • Successful completion of a certified licensed practical/vocational nursing program. 







  • Licensed to practice as a practical/vocational nurse in the state. 







  • Minimum of six (6) months to one (1) year of clinical experience. 







  • Six (6) months to one (1) year of previous home health or hospice experience preferred 







  • Reliable transportation. Valid and current auto liability insurance 







  • Must be a licensed driver with an automobile that is insured in accordance with state/or Company requirements and is in good working order. 







  • Must hold a current and valid CPR certification. 





 



ESSENTIAL JOB FUNCTIONS: 





  • Provides skilled nursing care to clients as directed by the registered nurse and physician’s plan of care in accordance with agency policies. 







  • Promotes and maintains client health through teaching and appropriate rehabilitative measures under the guidance of the registered nurse who is assigned to the same case. 







  • Assists clients in learning appropriate self-care techniques. 







  • Helps to achieve and maintain continuity of client care by assisting in planning and exchanging information with other health professionals. 







  • Prepares clinical progress notes and follows client care plans as developed by the registered nurse and according to the agency policy. 







  • Assist the physician and registered nurse in performing specialized procedures for which competency has been established. 







  • Prepares the equipment and materials for treatments observing aseptic technique as required. 







  • Keep abreast of changes and trends affecting nursing care and licensed nurse functions. 







  • Attends client conferences, meet with the quality improvement reviewer weekly and communicate regularly with the registered nurse. 







  • Must be capable of performing the job functions of this position with or without accommodations. 







  • Complies with acceptable professional standards and practice.   







  • Adhere to all of the Company’s policies and procedures. 







  • Perform other duties as assigned. 





  



Additional Duties   





  • Participates in on-call duties as defined by the on-call policy.   







  • Ensures that arrangements for equipment and other necessary items and services are available.    







  • Instructs, supervises, and evaluates home health aide care provided every fourteen (14) days.
