Job Description

We are looking for an enthusiastic and compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to become part of our clinical team. We are a trusted provider of comprehensive behavioral health services for residents in nursing homes throughout Plattsburgh, NY. This role is ideal for a practitioner who is deeply committed to geriatric mental health and motivated to enhance the emotional and psychological well-being of older adults.

Requirements

Position Responsibilities

  • Perform detailed psychiatric assessments and evaluations for residents in long-term care settings.

  • Develop and implement individualized treatment plans that reflect each patient’s needs and diagnoses.

  • Oversee medication management, including monitoring progress and adjusting treatments as clinically appropriate.

  • Coordinate care with interdisciplinary teams such as physicians, psychologists, social workers, and nursing staff to ensure a fully integrated approach.

  • Provide supportive counseling and psychotherapy to patients and, when appropriate, their families.

  • Educate residents and caregivers on mental health diagnoses, medication options, and long-term wellness strategies.

  • Maintain thorough, timely, and compliant documentation within the EHR system.

  • Participate in case reviews, team meetings, and initiatives focused on quality improvement and best practices.


Required Qualifications
  • Active New York Nurse Practitioner license with certification as a PMHNP.

  • Ability to independently manage a diverse patient caseload with strong clinical judgment.

  • Proven competence in psychiatric diagnosis, treatment planning, and psychopharmacology.

  • Strong interpersonal and communication abilities, with a focus on patient rapport and collaborative teamwork.

  • Proficient in electronic health records and general computer usage.

  • A compassionate, patient-first philosophy of care.
