Client Field Care Coordinator

Job Type: FULL_TIME

Location: Phoenix, AZ

Summary:

At Upscale Home Care, we are dedicated to providing personalized, compassionate care that supports dignity, independence, and peace of mind for our clients. As a Client Field Care Coordinator, you will be the first point of contact for clients and caregivers, responsible for supporting and managing our clients and caregivers. This role includes arranging schedules, resolving conflicts, reassigning caregivers as needed, handling client/caregiver inquiries, communicating with case managers, and providing general assistance with everyday questions.

Responsibilities:

• Conduct ATC visits with members to ensure their needs are met
• Respond to phone, email, and text inquiries in a timely and efficient manner
• Assist with new client setup and obtain authorizations
• Match and schedule caregivers with clients' needs
• Monitor caregiver schedules and adjust/verify missed or late clocks
• Provide compassionate and respectful support to clients and caregivers

Requirements:

• Outgoing, compassionate, tactful, detail-oriented, and able to multitask
• 1+ years of experience in a related field
• Ability to work in a fast-paced environment and prioritize tasks effectively

Compensation:

• Full-time, in-person, hourly position (based on experience)
• Sick time off after 90 days and PTO after 1 year of hire
• Paid legal holidays off
