Job Summary
As an LPN at Morristown Post Acute, you will be responsible for providing direct patient care to our residents, including administering medications, treatments, and procedures as prescribed by our medical team. You will also monitor patient conditions and report any changes to our medical team. Our ideal candidate is a motivated and caring individual who is passionate about delivering exceptional patient care and is committed to teamwork.
Responsibilities
Provide direct patient care to residents, including administering medications, treatments, and procedures as prescribed by the medical team
Monitor patient conditions and report any changes to the medical team
Assist with admissions, transfers, and discharges as needed
Collaborate with our interdisciplinary team to develop and implement individualized care plans
Maintain accurate and up-to-date patient records
Communicate effectively with patients, families, and healthcare team members
Requirements
Current LPN licensure in the state of New Jersey
High school diploma or equivalent required
