Job Title: Social Worker – Skilled Nursing Facility (Full-Time)
Location: Birmingham, Alabama
Facility: Redmont Health and Rehabilitation Center

Redmont Health and Rehabilitation Center, a 163-bed skilled nursing facility in Birmingham, Alabama, is seeking a Social Worker to join our team.

This is a full-time, on-site role supporting residents, families, and the care team to drive strong outcomes, smooth discharges, and high-quality care. We are looking for someone who is organized, compassionate, and able to operate in a fast-paced environment while keeping resident care at the center of everything.

What You’ll Do

• Lead and coordinate discharge planning to ensure safe and timely transitions
• Provide psychosocial support to residents and families
• Collaborate with nursing, therapy, and leadership on care plans and care conferences
• Support admissions, transitions, and resident assessments
• Connect residents and families with community resources and services
• Ensure compliance with state and federal regulations (including Centers for Medicare & Medicaid Services guidelines)
• Maintain accurate, timely, and compliant documentation

Qualifications

• Bachelor’s degree in Social Work or related field required
• Experience in skilled nursing, rehabilitation, or long-term care preferred
• Strong communication, organization, and problem-solving skills
• Ability to manage multiple priorities in a fast-paced environment
• Licensure not required
• Candidates with LBSW or LMSW are encouraged to apply
