Mountainview Licensed Homecare Agency is looking for a Registered Nurse DPS  to become part of an intricate, interdisciplinary team, to manage and coordinate patient care services and clinical personnel, in an assisted living facility. The ideal candidate,  must be detailed oriented and strong at multi-tasking and be knowledgeable of LHCSA regulations with supervisory experience. Additionally within regulatory standards, candidate, must be able to complete initial and reassessments of resident using the NYS UAS, develop individualized service plans, review and update service plans semiannually  or when a residents condition changes, communicate effectively with physician and facility discharge coordinators, assess residents need for referral to a physician or specialist, serve as a health educator for resident and staff as needed and perform supervision of residents and clinical staff as needed.

Job Requirements:

License as Registered Nurse in New York

Clinical experience in home care or an assisted living setting

Strong clinical assessment skills

Excellent nursing judgment and decision making skills
