Who You Are:

 

Amoskeag Health is seeking a dedicated Case Manager specializing in MAT services. You provide comprehensive case management, crisis services, and care coordination to children, families and adults served by Amoskeag Health. You thrive in working directly within the medical clinic as part of a multi-disciplinary team, providing ongoing case management services on-site, in the community, and in the home. 

 

 

What You'll Do:

  • Completes initial social assessment, annual health risk assessment and SBIRT screenings with all families and individuals served by Amoskeag Health.
  • Develops care plans with patient-centered goals for individuals and families, addressing risk and protective factors. Ensures that treatment goals are culturally competent, strength based, realistic and attainable.
  • Provides case management and care coordination as part of a multi-disciplinary team requiring effective and on-going communication and collaboration.
  • Provides care coordination for clients by acting as a liaison between various members of the team and as an advocate for patients with other service providers in the community.
  • Initiates crisis intervention and case management support services for patients where health, safety, social or emotional issues require assistance.
  • Regularly perceives potential problem situations and intervenes to offset adverse impact, demonstrates proactive attitude.
  • Acting as a mandated reporter, provides assessment and follow-up support to include protective services reports cases to appropriate agencies/authorities.
  • Documents all activities in patients records according to guidelines on quality and timeliness. This includes the completion of updates, CHAP+ plans and associated billing documented in the EMR.
  • Provide interventions to individuals as stated by their current care plan or in response to the daily needs of individuals identified through the Team Huddle process.
  • Assist clients and families in making informed decisions about program options and services provided by Medicaid, Medicare and insurance. Directs and assists clients with the resources to gain access to existing services such as public benefits, nutrition, housing, legal services, etc.
  • Ability to plan, organize, prioritize, delegate and accurately follow through in work activities with time constraints and interruptions to meet deadlines, working independently with a minimum amount of direction and/or supervision.
  • Works with the integrated MAT team to address vulnerabilities in social determinants of health.
  • Documents direct interventions with patients in the electronic medical record according to established protocols, policies, and procedures.
  • Completes brief SUD screening to aid in determining appropriate level of care and motivation. 
  • May be asked to represent Amoskeag Health at SUD related meetings in the community to promote programming, stay connected to SUD developments in the area, and make and maintain SUD organization relationships.

 

Requirements

 

What You'll Bring:

  • Bachelor's Degree or higher; Applicants without a bachelor’s degree may be considered if:
    • The applicant has an associate’s degree and at least 2 years’ experience working providing case management services; OR
    • The applicant has completed at least 50% of coursework required for a bachelor’s degree and agrees to complete the remaining coursework within a 3-year period.
  • Basic Life Support Certification (BLS) Required
  • Certified Recovery Support Worker (CRSW) certification is preferred
  • Three (3) years of community health and/or case management experience preferred
  • Must possess an understanding of grant-related requirements and be responsible for reporting and clinical tracking
  • Ability to work with technology such as computers, smart phones and/or tablets
