What You’ll Do:

  • Provide comprehensive case management and coordinate care plans for home-based clients.
  • Conduct client assessments, monitoring, and evaluations to ensure optimal care.
  • Collaborate with physicians, therapists, and caregivers to support client recovery.
  • Educate clients and families on disease management, medications, and care plans.
  • Ensure compliance with medical standards and maintain accurate client records.

Requirements

  • What We’re Looking For:

    • Registered Nurse (RN) license in Minnesota (required).
    • Experience in home health, case management, or clinical care (preferred but not required).
    • Strong communication and organizational skills with a client-centered mindset.
    • Ability to work independently while being part of a supportive team.
