Job Description

Delaware Hospice is actively seeking a full-time Dementia Care Navigator.



The Dementia Care Navigator is a key member of the Delaware Hospice Dementia Care GUIDE Program, serving as the primary non-clinical point of contact for individuals living with dementia and their caregivers. The role provides relationship-based caregiver support, care coordination, and navigation services using standardized workflows and technology-enabled tools, in alignment with the CMS Guiding an Improved Dementia Experience (GUIDE) Model.



The Care Navigator works collaboratively with the interdisciplinary team members to support high-quality, consistent dementia care delivery, caregiver engagement, and program scalability.
This is an exempt position.
