Position Overview:
We are seeking a dedicated Social Services Director (SSD/LSW) to lead our social services team. This role is responsible for ensuring residents receive outstanding support, care planning, and assistance navigating Medicare, Medicaid, insurance authorizations, and discharge planning. The ideal candidate thrives in a fast-paced healthcare environment and is passionate about improving residents’ quality of life.

Key Responsibilities:

  • Oversee the Social Services Department and provide leadership to the team.

  • Conduct resident assessments and develop comprehensive care plans.

  • Coordinate admissions, transfers, and discharges, ensuring compliance with state and federal regulations.

  • Serve as a liaison between residents, families, healthcare providers, and insurance companies.

  • Manage and maintain accurate medical records, including MDS documentation.

  • Address and resolve resident concerns and complaints promptly and professionally.

  • Participate in interdisciplinary team meetings to support holistic resident care.

Qualifications:

  • Licensed Social Worker (LSW) or equivalent SSD certification in Ohio.

  • Minimum of 2–3 years of experience in long-term care or skilled nursing facility social services.

  • Knowledge of Medicare, Medicaid, insurance authorizations, and discharge planning.

  • Strong leadership, organizational, and communication skills.

  • Ability to work independently and collaboratively in a team environment.
