Qualified Professional – Intensive In‑Home (IIH)



Youth Haven Services – Surry and Stokes County



Youth Haven Services is expanding our Intensive In‑Home (IIH) program, and we are seeking dedicated Qualified Professionals to join our team. This is an excellent opportunity for someone who is passionate about supporting youth and families, enjoys providing direct care and skill‑building services, and is ready to play a key role in helping clients achieve stability and success in their homes and communities.



Youth Haven Services is a comprehensive mental health agency specializing in serving children, adolescents, and families. We are currently hiring two Qualified Professionals to join our growing Intensive In‑Home (IIH) Program.



These roles focus on providing direct support, skill building, case coordination, and therapeutic interventions for youth with behavioral and mental health needs, all guided by their Person‑Centered Plans. Services are delivered in the home and community, following YHS and state guidelines.







Position: Qualified Professional (IIH)



Key Responsibilities




  • Complete initial intakes, Person‑Centered Plans, and submit authorizations/reauthorizations

  • Maintain accurate, up‑to‑date clinical records, including consents, intake packets, and required documentation

  • Complete and submit NC TOPPS, pre/post outcomes, discharge SARs, and other program paperwork

  • Coordinate care, scheduling, and person‑centered planning for assigned clients

  • Assist in identifying placements for clients and completing required placement documentation

  • Monitor caseloads, track authorizations, and verify insurance monthly

  • Provide strength‑based, recovery‑focused interventions

  • Support skill building, coping strategies, and community functioning

  • Perform other duties as required







Qualifications




  • Bachelor’s degree in a human services field (required)

  • Two years of documented experience working with youth/adolescents with mental health needs

  • Strong documentation and organizational skills, including ability to maintain logs and clinical records

  • Ability to teach skill‑building activities and support clients in home and community settings

  • Reliable transportation, valid driver’s license, and active auto insurance







Knowledge, Skills, and Abilities




  • Ability to engage youth and families in a supportive, therapeutic manner

  • Strong communication skills and the ability to collaborate with diverse populations

  • Ability to follow state service definitions and program requirements

  • Ethical, professional, and reliable work habits

  • Strong time‑management and problem‑solving skills







Work Environment




  • Home and community‑based service delivery

  • Travel required within Person, Alamance, and Caswell counties

  • Some remote tasks may be available based on program needs
