Description
As a HIV Prevention Coordinator, you will be a liaison for infectious and chronic disease prevention and education for the community. The HIV Prevention Coordinator will support the coordination and execution of Ending the HIV Epidemic outreach, education and testing activities, and the Project nGage research study, throughout Thrive Alabama’s service area by working in collaboration with community organizations and institutions in North Alabama to ensure availability of and access to PrEP and other services.
Requirements
Duties and Responsibilities
1. Identify community leaders and stakeholders for partnerships as applicable to grant objectives.
2. Develop, schedule and implement outreach events in service area, determining strategic community partners to deliver community-based outreach, education, and testing activities.
3. Connect individuals who test positive for HIV to medical care within established timeframes.
4. Coordinate referrals to healthcare providers, case management, mental health, housing, and other supportive services.
5. Follow up with clients to ensure successful engagement and retention in care.
6. Conduct HIV and viral hepatitis C rapid testing, to include culturally competent pre- and post-test counseling tailored to the client’s needs.
7. Educate individuals about PrEP, including benefits, eligibility, and adherence.
8. Leverage all department and agency resources to ensure comprehensive implementation plans are executed.
9. Track outcomes related to testing, linkage to care, and PrEP engagement.
10. Provide non-clinical education on HIV prevention, PrEP medication adherence, safer sex practices and risk reduction.
11. Recruit, screen, and refer individuals for Health Education programs and services.
12. Coordinate with the Director of Health Education to identify and establish memoranda of agreement (MOA) and memoranda of understanding (MOU) among the Health Education department and community partners.
13. Work collaboratively with PrEP Case Manager to identify client risk factors to address when engaging the community to promote awareness of PrEP program.
14. Provide technical assistance and training to peers and other professionals on all topics and related content addressed by the Health Education department.
15. Provide education around sexual health, HIV/AIDS, and biomedical HIV prevention services (e.g., PrEP and PEP) via presentations and tabling at local health fairs and other events in the community.
16. Provide tailored and culturally sensitive educational materials for targeted communities.
17. Assist with administrative tasks, including meeting management, project planning, and general communications as needed.
18. Design, plan, and implement assigned priority special projects.
19. Prepare special project workplans to outline timelines and deliverables for the assigned project, in collaboration with project teams, partners and department managers.
20. Keep the Director updated and informed on the status of individual projects and make recommendations regarding necessary project scope changes and problem solving as needed.
21. Prepare reports as required by funders and organizational leadership.
22. Maintain accurate records through data entry and monthly reporting and participate in annual grant meetings and audits.
23. Write assigned monthly/quarterly/annual grant reports.
24. Attend community-based meetings as assigned.
25. Attend and facilitate regular Health Education and staff development meetings.
26. Works with supervisor on promotion of events and outreach materials.
27. Assist in other functions or duties in the department as needed.
28. Participate in the agency Program Quality Improvement/Quality Assurance activities and initiatives.
29. Maintains working knowledge of national, regional, and local harm-reduction and HIV prevention activities, initiatives, and programs; maintains links accessing national and state HIV prevention program resources.
30. All other duties as assigned.
