Overall Function:



 



Will provide person-centered services to persons receiving substance treatment and/or case management services, to help them build supports and move towards recovery and community integration.



 



Essential Functions:



 




  • Works as a team member with a Drug and Alcohol Case Manager as part of the Community Engagement team to engage, coordinate, and refer individuals in the community to treatment and/or recovery support services.

  • As necessary, participates in all appropriate staff meetings and maintains beneficial dialogue with all members of partnering Emergency Medical Services (EMS) staff and CRS staff.

  • Works with clients and their families, involved in substance treatment and/or case management services, to develop and implement their Individual Recovery/Service Plans.

  • Participates in the after-hours on-call 24/7 treatment service access rotation process.

  • Offers guidance on the recovery experience, including stage-appropriate recovery education.

  • Assists clients in meeting their recovery goals by utilizing the phases of recovery: engagement, recovery initiation, recovery stabilization, and sustained recovery maintenance.

  • Educates consumers, family members, and professional staff about the recovery process; the damaging role that stigma can play in understanding recovery; and offers support related to obstacles that are often encountered during the early stages of recovery.

  • Provides individualized person-centered services/support to assigned clients.  

  • Provides vision driven hope and encouragement regarding opportunities for varying levels of involvement in community-based activities (i.e., work, school, relationships, physical activities, and self-directed hobbies.)

  • Facilitates access to various community resources to ensure basic needs are met.

  • Introduces and engages clients into the recovery community and monitors their recovery stability.

  • Provides ongoing assistance for individuals in obtaining and/or maintaining services from multiple systems (e.g., mental health, CYS, criminal justice, etc.) and offers referrals to treatment services when necessary.

  • Maintains current client case record activity and progress, prepares and submits all required program reports and documentation in an accurate and timely manner.

  • Adheres to all policies, laws, regulations, and codes of ethics, and confidentiality as outlined by federal and state laws and agency policies and procedures.
