As a Provider Services Nurse Practitioner, you will:
• Provide in-person and telehealth care across multiple care settings
• Diagnose and manage acute and chronic conditions
• Conduct patient assessments, prescribe medications, and guide treatment plans
• Collaborate with physicians, nurses, and community care teams
• Help patients receive the right care at the right time

What You’ll Be Doing
• Providing medical care across assisted living facilities, skilled nursing facilities, and patient homes
• Conducting patient evaluations, diagnosing conditions, and prescribing treatments
• Managing acute and routine medical conditions through in-person visits, phone calls, or telehealth
• Collaborating with physicians and interdisciplinary care teams
• Documenting clinical findings and coordinating care
• Building strong working relationships with community partners
Productivity Expectations
Our providers maintain a balanced clinical schedule designed to support both patient access and quality care.
Typical expectations include:
• ALF / home patients: approximately 40 visits per week
• SNF / rehabilitation patients: approximately 50 visits per week
Visit counts may vary depending on visit complexity and visit type.
Documentation is expected to be completed and signed within 7 days of the patient visit.
Providers participate in on-call rotation as needed.
What Helps You Succeed
• Master’s degree from an accredited Nurse Practitioner or Physician Assistant program
• Current Oregon licensure and prescriptive authority
• Ability to work collaboratively with physicians and interdisciplinary teams
• Strong clinical judgment and decision-making skills
• Excellent communication and organizational skills
• Valid driver’s license and reliable transportation
Providers must be comfortable traveling locally to serve patients and partner facilities.
