Job Description

Job Title: Registered Nurse (RN) Case Manager – Home Health | Clinical Autonomy & Leadership
Company: Home Health Care, Inc.
Location: Greater Twin Cities Metro Area (21-county service area)
Schedule: Full-Time, Monday to Friday, 8:00 AM – 5:00 PM, 1 weekend per month, 1 on-call shift per week
Compensation: $85,000 – $130,000 annually plus $6,000 in first-year bonuses

What You’ll Do
• Lead comprehensive nursing assessments and care planning.
• Manage a home health caseload across homes and community settings.
• Perform OASIS visits and coordinate interdisciplinary care.
• Provide education, guidance, and clinical direction to patients and families.
• Collaborate with physicians and care teams to drive outcomes.
• Maintain accurate, compliant documentation.

What We’re Looking For
• Active Minnesota Registered Nurse (RN) license.
• 3+ years of RN experience, home health preferred.
• Strong clinical decision-making and assessment skills.
• Confidence working independently in a field-based role.
• Organized, accountable, and patient-focused.
• Reliable transportation.
