Job Description
We are looking for a Physical Therapist to join our team with towns including Downtown Chicago, Lakeview, Edgebrook, Streeterville, Norridge, Harwood Heights, Humboldt Park, Skokie, Evanston, Rogers Park, and Lincolnwood.

Qualifications and Responsibilities:
  • Perform in-home physical therapy evaluations and treatments
  • Develop and implement individualized rehabilitation plans of care
  • Monitor patient progress and adjust treatment plans as needed
  • Educate patients and caregivers on exercises, mobility, and safety
  • Document visits accurately and efficiently in accordance with agency guidelines
  • Communicate patient progress with the interdisciplinary care team
  • Ensure compliance with clinical, documentation, and regulatory standards
  • Supervise PTA field staff as indicated
  • Licensed Physical Therapist (PT)
  • Doctorate in Physical Therapy (DPT)
  • Home Health experience preferred, but not required
  • Strong communication and documentation skills
  • ability to work independently in a home-based setting
  • professional, dependable, and patient-focused approach
  • Physical ability to perform in-home visits
