Job Description

Clinical Case Managers are licensed behavioral health clinicians who ensure that every participant in Salma Health’s Intensive Outpatient Programs receives seamless, person-centered care. Working closely with IOP Coordinators, therapists, and psychiatrists, they own the patient-engagement and continuity-of-care functions that underpin compliance, retention, and clinical outcomes.

Key Responsibilities:

  • Patient Engagement & Orientation: Conduct IOP orientations for new patients, introducing program structure, expectations, and resources. Use motivational interviewing and psychoeducation to enhance retention, reduce early drop-out, and sustain engagement.

  • Care Coordination & Discharge Planning: Lead discharge planning beginning at admission, ensuring linkage to outpatient providers, community resources, and payor-approved step-down services. Conduct collateral sessions with patients to support transition and continuity.

  • Indirect Patient Contact (IPC) & Compliance: Document collateral contacts, discharge coordination, and case management activities in compliance with CMS and commercial payor standards. Ensure IPC is properly captured for utilization review and reimbursement audits.

  • Group & Clinical Support: Co-facilitate psychoeducation or skills-based groups (e.g., DBT skills, relapse prevention). Provide clinical coverage for group sessions or therapeutic check-ins during staff absences, within scope of licensure.

  • Interdisciplinary Collaboration: Participate in weekly case conferences, contributing psychosocial and systems-level insights to treatment planning. Serve as liaison between therapists, psychiatrists, coordinators, and centralized navigation to ensure seamless care.

  • Outcomes & Quality: Track engagement, discharge, and readmission metrics; report trends to the Clinic IOP Director for quality-improvement initiatives. Implement corrective actions to improve retention, discharge quality, and community integration.

  • Safety & Crisis Support: Assist in updating safety plans and activating crisis escalation pathways as needed. Provide documentation that meets licensure, accreditation, and payor requirements.

Required Qualifications:

  • 3-5 years of experience working as a Clinical Case Manager as a Licensed practitioner

  • Current clinical licensure in the state of California.

  • Must physically reside in the state of California.

  • Strong working knowledge of evidence-based treatments for mood, anxiety, and trauma.

  • Proficiency with telehealth platforms and remote clinical work.

  • Excellent interpersonal, communication, and documentation skills.

  • In-tact Licensed Marriage and Family Therapist (MFT) or Licensed Clinical Social Worker (LCSW).

Preferred Qualifications:

  • Familiarity with acute psychiatric presentations and safety planning.

  • Experience providing case management services to an acute adult population

  • Comfortable working in milieu based clinical programs.

  • Previous experience working in an IOP, PHP, crisis residential and/ or in patient psychiatric hospital settings.
