(Direct Support Professional) POSITION DESCRIPTION:  

• Assist participant with needs identified in the Individualized Life Plan such as, self-care, toileting, showering, tooth brushing, grooming, dressing and ensure proper hygiene 

• Distribute and collect linen for laundry service in accordance with procedure. 

• Assist participant in attaining skills needed for daily living and socialization according to their abilities 

• Safely accompany participant to community resources (stores, places of worship, recreation, and health facilities, etc.) 

• Assist with meal preparation and serving of meals. 

• Responsible for cleaning of serving utensils and pots. 

• Complete Incident Reports as appropriate. 

• Complete room checks as needed. 

• Update logbook to foster greater and meaningful communication across shifts as needed. 

• Demonstrate the ability to work as part of a team, 

• Interact with clients to promote a friendly supportive but professional atmosphere, 

• All other duties as assigned by the Shift Supervisor or other supervisory staff. 

• Attending staff meetings and in-service training. 

• Participate in meetings as requested. 

• Other related tasks that may be assigned
