Description

SUMMARY:



To assist clients in acquiring permanent housing and provide case management.



 



RESPONSIBILITIES:




  1. Conduct apartment searches and escorts clients to lease negotiations with landlords.

  2. Make housing-related referrals such as furniture, housewares, maintenance, landlord/tenant, utilities, etc, and assist in apartment set-up.

  3. Design and implement individual client service plans according to specific timeframes outlined by the supervisor.

  4. Maintain current and accurate client charts and documentation as per standards outlined by the supervisor.

  5. Visit clients on a timely basis as per the program standard and make follow-up calls. Accompany clients with medical appointments and housing-related appointments.

  6. Schedule all field visits with the supervisor and provide documentation of visits within 2 business days.

  7. Attend ongoing case conferences and training.

  8. Refers client to internal and external service providers as needed for example legal, substance abuse, etc.

  9. Conduct housing workshops for housing readiness.

  10. Attend interdisciplinary meetings and weekly supervision.

  11. Work with supervisor in a cooperative and collaborative manner, including following directions and performing assigned duties.


Qualifications


Requirements:




  1. Bachelors degree and/or 5 years of social service experience.

  2. Some experience in case management and with the mentally ill chemically addicted population is preferred.

  3. Housing/real estate knowledge preferred.

  4. Must have a clean and valid driver’s license.

  5. The ability to establish workload priorities and balance diverse duties is needed.

  6. Must have excellent communication skills both orally and in writing, with individuals and groups regarding complex and sensitive issues.
