Job Title: Medicaid Eligibility Specialist (SNF / Long-Term Care)

Champion Care Wisconsin is hiring a Medicaid Eligibility Specialist to support our skilled nursing facilities across Wisconsin.

This is a key role focused on reducing Medicaid pending days, improving cash flow, and guiding residents and families through the long-term care Medicaid process. You will work closely with admissions, business office, and facility leadership to ensure applications are completed accurately and approved quickly.

If you have experience with SNF Medicaid, eligibility, and financial documentation, this is a high-impact opportunity to step into a role that directly supports both resident access to care and facility performance.


What You’ll Do

  • Manage and track Medicaid pending residents across multiple skilled nursing facilities
  • Lead the Medicaid application process from start to approval
  • Work directly with residents and families to gather financial documentation and verifications
  • Prepare, submit, and monitor Wisconsin long-term care Medicaid applications
  • Communicate with state Medicaid offices on application status, follow-ups, and determinations
  • Resolve eligibility issues, including spend-down and documentation gaps
  • Partner with admissions and business office teams to reduce Medicaid pending days
  • Educate families on Medicaid eligibility rules, asset limits, and spend-down requirements
  • Maintain accurate records to ensure compliance with state regulations

What We’re Looking For

  • Experience in a skilled nursing facility (SNF), long-term care, or healthcare setting
  • Strong knowledge of long-term care Medicaid eligibility and application processes
  • Background in SNF billing, business office, Medicaid coordination, or financial counseling
  • Ability to work directly with families to navigate complex financial situations
  • Strong organization, follow-up, and problem-solving skills
  • Ability to work across multiple facilities and teams
