Job Title: Nursing Support Specialist
Job Overview: The Registered Nurse, as part of the clinical team, will assist Care Coordinators and their supervisors with providing health promotion activities, referrals, assessments, and valuable expertise around the combination of behavioral health and physical health to improve the lives of the individuals we serve.
Work Location: Remote, must reside in Tri-State area.

- Knowledge of developmental disabilities, chronic disease and social determinants of health.
- Ability to engage people with behavioral health needs.
- Ability to teach and lead others while providing medical expertise.
- Ability to speak and convey medical information to groups of people.
- Knowledge of person-centered planning regulations.
- Demonstrates effective leadership skills.

- Demonstrates ethical and professional responsibilities and boundaries.
- Demonstrates capacity to use Health Information Technology to facilitate communication.
- Knowledge of confidentiality regulations/HIPPA.

- Demonstrate a comfort level of engaging with people with behavioral health needs.
Experience:
- RN with at least 2 years OPWDD experience.
- Strong computer knowledge.
- Familiarity with person-first, person-centered planning concepts.
Essential Responsibilities:
- Serve as part of the Clinical Team for Person Centered Services.
- Design, implement, and present trainings as needed.
- Review and analyze data and trends.
- Construct/implement policies and procedures.
- Assist with crisis intervention and incident management.
- Facilitate collaborative/consultative relationships with agency departments and community partners.
- Interact with people with developmental disabilities and their families.
- Provide monitoring of physical health information at various levels throughout the agency.
- Serve as a consultant in situations where medical expertise is required.
- Implement interventions following the nursing process.
- Provide education on medical issues and concerns to individuals and their families.
- Provide case review or audit of individuals with significant medical issues or behavior issues.
- Participate in mandatory staff meetings.
