Responsibilities:

• Conduct assessments to understand residents' social and emotional needs.
• Develop and implement personalized care plans addressing residents' physical, social, and emotional well-being.
• Provide counseling and emotional support to residents and families.
• Collaborate with the interdisciplinary team on resident care plans.
• Advocate for residents' rights, ensuring their safety and well-being.
• Maintain accurate resident records.
• Participate in quality improvement initiatives.

Qualifications:

• Master's degree in Social Work preferred. (We value relevant experience!)
• Long-term care experience a plus.
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively.
• Strong organizational and time management skills.
