This is a pay per visit position! Currently looking for part-time, with the potential to become full-time.

🏠 Position Summary



We’re seeking five full-time Registered Nurses (RNs) to join our Dublin, Ohio team!

As a Home Health RN, you’ll provide one-on-one skilled nursing care in patients’ homes, coordinate with physicians and care teams, and ensure top-quality outcomes for every patient.







🩹 Key Responsibilities



✅ Conduct initial and ongoing assessments — including Start of Care (SOC), Recertifications, Resumptions, and Discharges

✅ Develop, implement, and evaluate individualized plans of care

✅ Administer medications, wound care, and treatments

✅ Educate patients and families on care routines, medications, and disease management

✅ Collaborate closely with physicians, therapists, and the interdisciplinary team

✅ Document care accurately in WellSky EMR (or similar)

✅ Ensure compliance with Medicare, Medicaid, and agency standards

✅ Participate in weekend rotation as needed
