Social Services Coordinator
Location: 3935 Sunnyside Drive, Harrisonburg, VA 22801
Schedule: Full-Time, 8:00 a.m. - 4:30 p.m.
The Social Services Coordinator is responsible for supporting residents by identifying social, emotional, and mental health needs and coordinating services and resources to address those needs. This role provides guidance and support to residents as they navigate life changes and challenges by offering appropriate resources, solutions, and coping strategies. The Social Service Coordinator works closely with the Social Services Director, residents, and families to ensure the residents’ needs are being met.
Qualifications:
- Bachelor’s degree in Social Work or a related human services field required.
- One to three years of related experience preferred; healthcare experience strongly preferred.
- Ability to accurately assess information, make sound decisions, and take appropriate action based on available information.
- Ability to handle confidential information with professionalism and discretion.
- Strong written and verbal communication skills, including professional phone etiquette and effective grammar usage.
- Strong organizational skills with the ability to multitask while maintaining attention to detail.
- Proficient in the use of computers, printers, copiers, and other office equipment; working knowledge of Microsoft Office and electronic medical records systems required.
- Strong customer service and interpersonal skills with a hospitality-focused approach when interacting with residents, families, staff, and visitors.
Essential Job Duties:
- Complete MDS assessments and care plans in collaboration with the interdisciplinary team (IDT), ensuring timely and accurate completion.
- Develop and maintain social services care plans for residents, including behavior interventions, discharge planning, and advance directives.
- Submit equipment orders to DME suppliers and ensure delivery prior to resident discharge.
- Coordinate and submit home health referrals and orders to the resident’s agency of choice prior to discharge.
- Coordinate interdisciplinary care plan meetings and ensure families receive invitations and scheduling information.
- Attend and participate in QAPI and UR meetings as part of the interdisciplinary team to support resident care and compliance initiatives.
- Communicate effectively with residents, families, staff, and external parties to provide information and services that support resident well-being.
- Provide support and services to residents with mental and physical disabilities in a compassionate and professional manner.
- Facilitate resident shopping needs by approving withdrawals from personal fund accounts and coordinating the purchase and return of personal items and clothing.
- Collaborate with staff, residents, and families to ensure residents have necessary personal items and clothing; obtain family permissions when required.
- Coordinate with the billing office, transportation department, and POAs regarding delivery and forwarding of resident mail; assist with related questions and concerns.
- Coordinate with residents’ families regarding bed holds, including notification, authorization, and documentation requirements.
- Maintain and update in-house waiting lists for resident room assignments and transfers.
- Assist residents with voter registration, voting processes, and absentee ballots as needed.
- Investigate complaints and maintain knowledge of applicable state and federal regulations; complete required reporting to regulatory agencies.
- Provide education and training to staff regarding residents’ rights, abuse, and neglect, including new hire orientation and ongoing in-service training as needed.
- Cross-trained in all aspects of Social Services and Admissions and able to provide departmental support as necessary.
- Other duties assigned.

Work Environment & Physical Requirements
Work environment: Work is performed in a general office environment with standard lighting, comfortable temperatures, good ventilation, air quality, and minimal noise levels. Physical demands: Ability to lift, carry, push, or otherwise move equipment and office materials weighing up to 25 pounds using proper lifting and moving techniques. Work Hazards: Minimal hazards consistent with a general office environment.
