Key Responsibilities:
Accurately manage and order medications, supplies, and medical equipment essential to resident care
Deliver individualized nursing care plans with precision and empathy, tailored to each resident’s unique needs
Perform wound care, treatments, and clinical procedures with attention to detail and best practices
Maintain thorough and timely documentation of all resident care activities, assessments, and outcomes
Safeguard resident privacy and ensure strict adherence to HIPAA and regulatory standards
Qualifications:
Active and valid LPN license in the State of New York
Prior experience in a skilled nursing or long-term care setting preferred
Strong commitment to compassionate, patient-centered care and professional nursing standards
