Responsibilities:
- Carry out the health care plan designed by the Practitioners
- Administer treatments and/or medication prescribed by Practitioners
- Interview clients for the purpose of gathering health history
- Administer and evaluate medical tests as appropriate
- Provide health teaching for patients and their families
- Assess patients telephonically and in-person for acute physical issues
- Act as liaison between prescribers, consumers, laboratories, PCPs, other healthcare providers, pharmacies and staff to ensure continuity of care
- Provide support and guidance to patients in maintaining a therapeutic environment
- Promote Chestnut’s culture of customer service excellence through adherence to behavioral standards
Qualifications:
- High school diploma or equivalent required
- Active Illinois RN or LPN license required
- Knowledge of pharmacology and possible medication side effects required
- CPR certification required (or ability to obtain within 90 days of hire)
- Basic computer skills required, including MS Word, Outlook, e-prescribing, and EMR systems
- Must successfully complete Chestnut’s background check process
Compensation
The anticipated starting pay for this position ranges from $19.85 to $40.84 per hour, depending on licensure (LPN or RN), experience, and qualifications.
Chestnut considers multiple factors when determining starting pay, including skills, education, experience, local market conditions, and internal equity.
Opportunities for additional earnings may include shift differentials and bonuses, where applicable.
