Job Description

Location/Department:  Hospital - Pharmacy

PRN

Job Summary:

The registered pharmacist provides pharmacy services to MMH with an emphasis on clinical monitoring and therapeutic interventions with a goal to improve patient outcomes. The pharmacist will advocate for safe and effective dosing of medications and promote a culture that continuously improves the medication use process.

Supervisory Responsibilities:  None

Duties/Responsibilities:

  •          Demonstrates awareness of patient rights.
  •          Uses respect & dignity when dealing with death and dying.
  •          Understands procedure to clarify physician orders.
  •          Uses standing orders when indicated.
  •          Reports sentinel events or adverse drug reactions or incidents as warranted.
  •          Uses only approved abbreviations.
  •          Understands patient safety goals & actions taken.
  •          Verifies &/or reviews orders entered by other professionals.
  •          Procures the correct drug needed by a department or patient.
  •          Accurately enters & documents drug distribution with the required EHR information.
  •          Attends and participates in multidisciplinary rounds.
  •          Adjusts doses to age, weight, renal status, disease & documents as appropriate in EHR.
  •          Procures unusual, non-familiar drugs as required.
  •          Accurately fills Omnicell as required.
  •          Properly prepares sterile IV products to USP 797 and USP 800 requirements w/ documentation as required.
  •          Properly prepares non-sterile compounds to USP 795 requirements
  •          Checks all medication prior to final dispensing
  •          Assures compliance with all state, federal & local laws & regulations.
  •          Plans & organizes the day’s operations.
  •          Oversees pharmacy techs & volunteers.
  •          Prevents, corrects & documents medication errors.
  •          Resolves interdepartmental issues in a professional manner.
  •          Complies with the approved drug formulary & policies/procedures.
  •          Confirms correct drug use & storage (DUE).
  •          Helps perform the annual drug inventory.
  •          Renews one’s license with the required hours of continuing education credit before the license expires; presents licensing documentation to manager when received.
  •          Teaches staff members to help increase skills & competence.
  •          Provides clinical services up to the limits of one’s competence level & documents same.
  •          Utilizes correctly & effectively the clinical resources available to Pharmacy dept.
  •          Maintains awareness of the contents of Pharmacy’s policy & procedure manuals & notebooks.
  •          Provides pharmacy services at both the main hospital and the Cancer Center.
  •          Demonstrates competence in technician functions in addition to pharmacist functions and assists technicians when necessary.
  •          Participates in and supervises the receipt, dispensing and distribution of controlled substances.
  •          Ensures controlled substance inventories are accurate and participates in resolution of controlled substance discrepancies.
  •          Works rotated weekends and holidays as scheduled.

  • Must acquire basic knowledge of drugs, dosages, usage, reference sources & other pertinent pharmaceutical information; utilizes pharmacy math skills for dose calculations.
  • Able to concentrate/focus on work in a small, sometimes noisy/distracting environment.
  • Able to operate Laminar flow hood or newer models, pharmacy dispensing devices, other new equipment as purchased.
  • Able to operate basic office equipment & hospital applications.

Education and Experience:

  • Bachelor’s degree majoring in Pharmacy required – graduate from ACPE accredited pharmacy college.
  • Must be a registered pharmacist with the Indiana Board of Pharmacy.
  • No prior work experience required.

Physical Requirements:

  • Constant use of hands & manual dexterity; frequent sitting; occasional walking, pushing, pulling, climbing, stooping, kneeling, standing, reaching, & talking.
  • Occasionally may need to lift between 25 lbs to 50 lbs.
