Job Description

What to expect.

  • In collaboration with the patients' interdisciplinary treatment team, performs a full range of aftercare and discharge planning services geared to mentally ill patient population. 
  • Maintains comprehensive knowledge base of agencies, organizations, and resources within the referral community for discharge planning and placement. 
  • Communicates with patients based on an understanding of the developmental needs of the population. 
  • Develops and assists in the implementation of discharge and aftercare plans; works with patients and families to facilitate the community transition and assure appropriate match to patients' needs.

Pay for this position is determined on a number of factors, including but not limited to, years and level of related experience.

What we need from you.

  • Bachelor's degree in a related mental health care field, such as Social Work, Nursing, Psychology, or a Rehabilitation Services specialty. 
    • For internal candidates, high school education plus an additional 5 years of relevant experience (2 years of work must be within Sheppard Pratt) may be considered a substitute for a bachelor’s degree.
  • Knowledge of psychiatric terminology, discharge and aftercare planning methods, and community referral resources - acquired through six to twelve months of relevant field or work experience in a psychiatric setting.
