As a Case Manager at Horizon, you will…
- Become familiar with the SBIRT public health model.
- Work with students and families to determine wraparound services that may complement the adolescent/young adult’s treatment and support the family unit. These may include:
- Crisis Outreach
- Transportation Assistance
- Housing Assistance
- Food Assistance
- Residential Treatment
- Respite Care
- Recreation Therapy
- Other Medical Care and Health Services
- Legal Assistance
- Advocacy
- Social Services
- Education Assistance
- Tutoring
- Financial Aid
- Support Groups
- Family Support
- Maintain continuous contact with SBIRT team, adolescents/young adults, and families so that everyone is informed about a participant’s’ full spectrum of services.
- Ensure that all patient data and information is properly recorded, entered, and stored.
- Ensure complete participant confidentiality at all times.
- Conduct grant required surveys and data entry for all participants at intake and program completion
- Assist Project Coordinator with intake and referral tasks as needed
- Participate in relevant trainings throughout the course of the program.
- Support program marketing efforts including community outreach and tabling events
- Travel to schools/partners/community sites as needed.

- Bachelor's degree in related field preferred.
OR
- Associate's degree with 3 years of experience in the behavioral health field required.
OR
- High school diploma with 6 years of experience in the behavioral health field required.
Location
This position will be located at 699 Hertel Ave, Buffalo NY. Local travel may be required as needed.
Hours
This is a full-time position from Monday - Friday.
Compensation
Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $23.45 - $25.40. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.
*Please note: This is a grant-funded position.
Physical demands associated with this position include:
- Sitting for extended periods, alternating between standing and sitting
- Mobility required with repetitive wrist, hand, and finger movements
- Handling light duties, occasionally lifting objects up to 20 pounds
