The Bilingual Care Coordinator, Oral Health will work closely with the project team, and support the integration of medical and dental services to improve patient care and outcomes. This position is ideal for an organized, detail-oriented individual who is passionate about healthcare innovation and improving access to coordinated care for patients.

This position offers the opportunity to gain hands-on experience in integrated care delivery, working closely with clinicians, care coordinators, and administrative staff to drive meaningful improvements in oral healthcare access and outcomes. Strong organizational skills, attention to detail, and excellent communication abilities are essential for success in this role.

The Oral Health Program is a HRSA- funded national initiative that seeks to improve oral health for children aged 6m-5y. The goal of the program is to increase rates of preventive oral health services among children 6m to 5y who have not had dental care in the last 6 months by improving medical-dental integration. The Community Health Center, Inc. was awarded a grant to be part of this important initiative.

Key Responsibilities:

Duties include, but are not limited to, the following:

Care Coordination Support

  • Serve as a point of contact between medical and dental teams to streamline patient care pathways.
  • Coordinate patient referrals between primary care and dental providers, ensuring timely follow-up.
  • Maintain tracking systems for patient appointments, referral outcomes, and care plans.
  • Identify and address barriers to patient access, such as transportation, insurance, or language needs.

Oral Health Education

  • Educate caregivers and community members on the importance of oral health for children aged 6 months to 5 years.
  • Partner with community organizations and clinics to deliver educational sessions and outreach activities.

Stakeholder Engagement

  • Collaborate with clinical staff, care teams, and community partners to strengthen integration workflows.
  • Participate in regular interdisciplinary meetings to review cases, discuss challenges, and align efforts.
  • Support patient engagement strategies, including educational materials and outreach.
  • Build relationships with community organizations to connect patients with supportive resources.

Operational & Administrative Tasks

  • Assist in aligning workflows with compliance and reporting requirements.
  • Maintain accurate, confidential records of coordination activities in electronic health systems.
  • Provide logistical support for training sessions, workshops, and stakeholder meetings.
  • Contribute to process mapping and the development of standardized care coordination protocols.

Continuous Improvement

  • Gather feedback from patients, families, and care teams to identify opportunities for improvement.
  • Research and share best practices in integrated care coordination.
  • Participate in professional development opportunities related to project management and integrated care.

Qualifications:

  • Associate degree required, Bachelor’s degree in human services, Public Health, Social Work, Community Engagement, or a related field preferred.
  • Bilingual (English/Spanish).
  • Excellent communication, interpersonal, and organizational skills.
  • Demonstrated ability to engage effectively with diverse and vulnerable populations.
  • Ability to work collaboratively with providers, community partners, and multidisciplinary teams.
  • Strong organizational skills with exceptional attention to detail and the ability to prioritize multiple tasks and deadlines.
  • Proficiency in Microsoft Office Suite (Excel, Word, PowerPoint, Outlook).
  • Demonstrated maturity, flexibility, and compassion when handling complex or sensitive situations.
  • Knowledge of local community resources, target populations, and health equity principles.
