We are seeking multiple experienced and compassionate Psychiatric Mental Health Nurse Practitioners (PMHNPs) to provide psychiatric care in long-term care, skilled nursing, nursing home, and post-acute care settings throughout Tompkins County & surrounding area. Requirements
Perform comprehensive psychiatric evaluations for residents in long-term care and skilled nursing facilities
Provide ongoing medication management and psychiatric follow-up visits
Diagnose and treat common geriatric psychiatry conditions including depression, anxiety, dementia-related behaviors, psychosis, mood disorders, and chronic mental illness
Prescribe, monitor, and adjust psychotropic medications based on each resident’s clinical presentation
Develop individualized treatment plans that align with residents’ psychiatric, medical, and psychosocial needs
Collaborate with nursing staff, physicians, medical directors, social workers, therapists, and interdisciplinary care teams
Provide behavioral health recommendations to facility staff to support continuity of care
Maintain timely, accurate, and compliant documentation within the medical record system
Communicate effectively with residents, families, guardians, and facility teams
Support quality psychiatric care delivery within the nursing home / LTC setting
Qualifications:
Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
Active and unrestricted New York State Nurse Practitioner license
DEA registration preferred or ability to obtain
Strong clinical judgment and ability to practice independently
Experience working with geriatric, elderly, long-term care, skilled nursing, nursing home, behavioral health, psychiatry, or post-acute populations preferred
Prior experience managing dementia, behavioral disturbances, chronic mental illness, or psychotropic medication regimens strongly preferred
Excellent communication and collaboration skills
Must be comfortable working in skilled nursing facilities / nursing homes / LTC environments
