Job Description

Rehab Services Coordinator, Full-Time
Location: Manhattan Office, Mon-Fri 9a-5p

 

JOB SUMMARY:

To provide administrative and clerical support to the Rehab Services department, staff, Rehab Director and Chief Clinical Rehab Officer.

 

QUALIFICATIONS:

  • High school diploma or equivalent, some college preferred.
  • Minimum of two years working in a busy environment using the telephone, fax, computers, scheduling modules and filing.
  • Ability to handle multiple clerical tasks simultaneously and stay organized.
  • Computer literate, proficiency in scheduling systems.
  • Strong communication skills and ability to communicate effectively with healthcare professionals.

 

RESPONSIBILITIES:

  1. Printing emails received from the Intake Services department for the assigned boroughs to verify which cases need placement of a PT, OT, or SLP.

 

  1. Responsible for the daily placements of PT, OT, & SLP immediately upon receiving initial requests from the Intake department.

 

  1. Providing follow-up communications to the Case Manager informing them of whom the therapist is who will conduct the evaluation and/or SOC.

 

  1. Receiving follow up notes from the COC with service change request to generate PT, OT, and/or ST order for continuation of service.

 

  1. Responding to the Follow–Ups from case managers, clinical manager, and rehab managers regarding therapy placements or service interruption.

 

  1. Notifying the assigned designated rehab manager for the assigned borough with the therapist’s interactive schedule variances.

 

  1. Printing all outstanding and weekly reports for PT, OT & SLP staff with open visits and assignment of correct therapist to patient orders.

 

  1. Printing therapist's schedules of “assigned” visits report that have not been completed and calling therapist to get an update as well submitting rehab notes in a timely manner.

 

  1. Data entry of required information into McKesson systems.

 

  1. Logging all cases with the therapist's name and the patient's name on the Rehab placement log and filing as appropriate.

 

  1. Providing customer service to all therapists.

 

  1. Providing full coverage of duties for other Rehab Services administrative/clerical staff that are out of the office.

 

  1. Maintain accurate record keeping in a confidential manner, as per agency policies and procedures

 

  1. Notify supervisor of obstacles to job performance

 

  1. Report all the suspected violations to supervisor, Compliance Officer or Compliance hotline

 

  1. Observe Client’s rights and confidentiality in accordance with HIPAA compliance.

 

  1. Performs other duties as required.
