Job Description

We are seeking for a skilled and compassionate PMHNP to join our expanding behavioral health team. We collaborate with nursing facilities across Orange County NY, to provide exceptional psychiatric care to older adults. This position is ideal for a clinician who is enthusiastic about geriatric mental health and committed to improving the quality of life for senior residents.

Requirements

Role Overview

  • Conduct detailed psychiatric evaluations and mental health assessments for residents in long-term care environments.

  • Diagnose mental health conditions and develop individualized treatment plans tailored to each patient’s clinical needs.

  • Oversee medication management, track patient response, and revise treatment strategies when necessary.

  • Partner with an interdisciplinary team of psychologists, physicians, nurses, and social workers to ensure well-coordinated, whole-person care.

  • Deliver therapeutic interventions, including counseling and psychotherapy when appropriate.

  • Educate patients and their families on psychiatric conditions, treatment expectations, medication compliance, and wellness practices.

  • Maintain thorough, accurate, and compliant documentation within the EHR system.

  • Participate in interdisciplinary meetings, case consultations, and quality improvement efforts that support clinical excellence.


Candidate Requirements

  • Active New York State Nurse Practitioner license with PMHNP certification.

  • Ability to manage a caseload across multiple facilities independently.

  • Strong diagnostic skills and a solid understanding of psychiatric medications, therapies, and clinical best practices.

  • Exceptional communication and interpersonal skills with a patient-centered approach.

  • Proficiency with electronic health records and standard computer tools.

  • Empathy, professionalism, and a commitment to delivering high-quality mental health care.
