Position Summary

The Community Based Therapist will provide individual/family counselling in the home/community settings as well as group therapy in an office setting when appropriate. Individual will work closely as the "Team Lead" with Certified Peer Specialist, Psychiatric Rehabilitation Counselor and the Service Coordinator (Mobile Transition Age Youth (MTAY) Treatment Team). The therapist will use evidence- based practices and assessment tools designed specifically for transition age youth.

Essential Responsibilities

  1. Individual, family, and group therapy.
  2. Function as "Team Lead" with MTAY Treatment Team.
  3. Assessment and treatment planning.
  4. Intensive service delivery.
  5. Crisis Intervention & safety planning.
  6. Work collaboratively with internal programs and external stakeholders.
  7. Participate in professional development activities including supervision, consultation, and training.
  8. Promote wellness and recovery in all aspects of service delivery.
  9. Documentation in electronic health record.

Qualifications

  • Clearances. State Police; FBI clearance; child abuse clearance; CPSL Mandated Reporter-Recognizing and Reporting Child Abuse training; any additional background checks/clearances required by state governing bodies.
  • Valid driver’s license and current vehicle insurance.

Working Conditions

  • Physical Demands. Requires vision, speech, and hearing. Position allows employee to move about freely or sit at will when performing job duties.
  • Environmental Factors. Office, home, community.
  • Working Hours. Non-traditional hours as determined by the program, including some evening hours and occasional weekends and on-call duties.
