Greenfield Rehabilitation Agency is expanding and seeking passionate Occupational Therapists (OTRs) to join our exceptional team at our senior campuses in Lancaster, while also supporting communities in Prairie du Chien. At Greenfield Rehab, we provide functional, effective, and rewarding occupational therapy across skilled nursing facilities, senior communities, assisted living environments, and in-home care.
This is a unique opportunity to combine hands-on clinical care with flexible scheduling—whether you’re seeking full-time, part-time, or PRN work—while making a real difference in the lives of older adults across multiple communities. Full-time candidates are eligible for a sign-on bonus of up to $6,000.

Occupational Therapist (OTR) Responsibilities
Complete functional and cognitive evaluations
Develop individualized, resident-centered treatment plans
Provide skilled OT interventions to promote independence and quality of life
Educate residents, families, and staff on adaptive strategies and safe mobility
Recommend assistive equipment and environmental modifications
Lead therapeutic activities to enhance coordination, strength, and functional abilities
Collaborate with nursing, PT, SLP, and interdisciplinary teams
Maintain accurate and timely clinical documentation
