Job Description
SUMMARY:nProvide quality and cost effective primary care services to patient population, meeting their diverse health and social needs.nnThe Chautauqua Center (TCC) expects all employees to respond and participate to emergency situation per emergency policies and procedures.nnDUTIES AND RESPONSIBILITIES:nn1. Clinicalna. Per specialty, perform complete examination of patients including adults, children and infantsnb. Assess and manage acute and chronic conditions and participate in disease registrync. Screen all patients for health maintenance needs, including immunizationsnd. Educates patients and family members in health promotion, disease prevention and birth control methods as appropriatene. Assists in preparation and selection of patient education materialsnf. Initiates diagnostic studies referring abnormal results of concern to Medical Director of specialistng. Helps coordinate efficient flow of patients through the system of carenh. Helps monitor status of referralsni. Obtain and review patient histories and develops patient care charts, ensuring completeness and accuracynj. Helps facilitate creation and tracking of patient data, specimens and recordsnk. Practices safety, environmental, and/or infection control methodsnl. Helps provide triage services and treatment services during disaster, per protocolnm. Maintains hospital privileges as designated by TCCnn. Shares a call schedule with other providersno. Other duties as assigned by TCCn2. Managementna. Provide a proportionate share of daily supervision and be able to act as mentor to PA/NP and co-sign their chartsnb. Provide consultative support, including differential diagnosis to NP, PA, mid-level Providers, triage nurse and other Physiciansnc. Participate in proctoring of new hire and newly graduated Providersnd. Other duties as assigned by TCCn3. Administrativena. Provide trainings on procedures, risk management and emergency carenb. Participate in Quality Assurance, peer review and maintain confidentialitync. Participate in quality improvement, chart audits, and process improvement programs both system wide and cluster specificnd. Participate in cluster specific meetings and administrative task as assigned by Medical Director or cluster lead Cliniciansne. Helps in recording of no-show visits and lab results/refillsnf. Participates or collaborate with marketing and other health education staff on education and patient recruitment health fairsng. Maintain current knowledge-base and appropriate licensurenh. Performs miscellaneous job-related duties as assignedni.
