Job Position Description: Responsible for case management/family engagement, including providing community resources and collaborations, support service delivery, monitoring and reporting, and providing best practices in working with families.
Pay $15.00 - 19.19 -Depends on education
Responsibilities:
- Process eligibility applications per systems, procedures and policies established by the agency.
- Assist in coordinating/participating in recruitment efforts to help ensure ample wait list selection for site.
- Form and maintain working relationships with families including providing support and coordination for families with required procedures: enrollment, orientations, screenings, and child assessments.
- Provide resources and referrals to families as requested to support their efforts and family partnership plans.
- Document in the agency data system the support provided and interactions with families.
- Form and maintain working relationships with outside agencies to facilitate both recruitment and services to enrolled and wait listed families.
- Associates or Bachelor's degree in Social Services related field
- Positive, motivating and energetic personality
- Professionalism and can be flexible in a changing environment
- Intermediate to advanced computer skills, database, internet and e-mail.
- Must be able to effectively operate a motor vehicle, as local travel is required.
- Able to work with minimal supervision, including balance and prioritizing work load.
- Regular repetitive motion with hands, computer keyboarding, phone
