Home Health Licensed Practical Nurse LPN

Job Summary:

Pathwell Health is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team of home health care professionals in Loudoun County, Virginia. As a Home Health LPN, you will play a vital role in providing skilled nursing care to patients in their homes, under the direct supervision of our Clinical Supervisor. Essential Duties and Responsibilities:

  • Provide skilled nursing care under the direct supervision of the Clinical Supervisor

  • Complete evaluation tasks, including vital signs and medication reviews

  • Coordinate with physicians, therapists, and other individuals in the client's care plan

  • Assess and chart observations of the client's condition at each visit

  • Maintain complete and accurate documentation of nursing care

  • Work collaboratively with all members of the healthcare team

  • Provide education to clients and families on proper home healthcare procedures and strategies

  • Record patients' medical information and vital signs

  • Monitor, record, and report symptoms or changes in patients' conditions

  • Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans

  • Modify patient treatment plans as indicated by patients' responses and conditions

  • Monitor all aspects of patient care, including diet and physical activity

  • Assess the needs of individuals, families, or communities, including assessment of individuals' home or work environments, to identify potential health or safety problems

  • Perform other duties as assigned

Minimum Qualifications:

  • Associate's degree required; Graduate of an accredited state-approved, Bachelor's Degree from a school of nursing preferred

  • Current and valid Virginia Licensed Practical Nurse license

  • One year of LPN experience

  • Current CPR certification

  • Driver's License

  • Car Insurance

  • Car Registration

  • Strong interpersonal skills that build personal rapport and establish trust with the client

  • Computer literate

  • Strong organizational skills

  • Excellent written and communication skills

  • Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers)

  • Home health care experience

  • Bilingual (Spanish) preferred

Salary: $60,000 - $80,000 ANNUALLY
