Summary:

The Occupational Therapist Assessor for LIFE is responsible for assessing referred community clients in their home setting via the OT Initial assessment form and the NYS Safe at Home eligibility criteria to determine eligibility to be enrolled into a Program of All-Inclusive Care for the Elderly (PACE) program. Additionally, the OT assessor is responsible for assessing areas of need to determine services to be provided upon enrollment, the creation of the initial plan of care, entering documented diagnosis, Initial OT orders, authorizations and other assessments recommended to assess safety in the community. This position will also work collaboratively with other members of the enrollment and leadership team at LIFE to promote a streamlined enrollment process.


Responsibilities:

EDUCATION:

  • Graduate of a B.S. or M.S. degree program in OT required
  • State of New York license and registration required
  • Knowledge of rules and regulations governing Licensed Home Health Agencies preferred

EXPERIENCE:

  • One (1) year experience as an OT completing home based care required
  • Minimum one (2) years relevant experience working with the geriatric population preferred
  • One year experience with the frail elderly preferred
  • Knowledge of NYS Safe at Home Criteria preferred

KNOWLEDGE, SKILL AND ABILITY:

  • Must possess a valid driver’s license and have a car available for work use
  • Must demonstrate the ability to complete a home safety assessment
  • Must have the knowledge to adapt and modify the home setting
  • Strong written & verbal communication skills
  • Computer skills required, i.e., software appropriate to position, word, excel
  • Ability to prioritize
  • Strong assessment skills required
  • Ability to multi task
