We are seeking a dedicated and passionate Family Nurse Practitioner (FNP) to join our team remotely as a full-time/part-time 1099 contractor. This role offers the flexibility to set your own hours and work from the comfort of your home while making a significant impact on improving health disparities. Multi-state licensure is preferred.


**Key Responsibilities:**

- Conduct virtual primary care evaluations and assessments.

- Manage and oversee care for chronic diseases, acute care needs, and routine health maintenance.

- Develop and implement individualized treatment plans.

- Collaborate with a multidisciplinary team to provide holistic care.

- Maintain accurate and timely documentation in our electronic health records system.

**Qualifications:**

- Active FNP certification.

- Licensure in multiple states is preferred.
