Position Summary
We are seeking a compassionate and motivated Social Services Assistant to support our Social Services Department. This role plays a key part in assisting residents and families with psychosocial needs, discharge planning, and care coordination in a long-term care / nursing home setting.
Key Responsibilities
- Assist the Social Services Director in providing social services support to residents and families
- Help coordinate discharge planning and transitions of care
- Support residents with psychosocial needs, emotional support, and adjustment to facility life
- Maintain accurate documentation in accordance with CMS and Florida state regulations
- Participate in care plan meetings and interdisciplinary team (IDT) discussions
- Assist with resident advocacy, grievance processes, and family communication
- Coordinate community resources and referrals as needed
- Ensure compliance with HIPAA and resident rights regulations
Qualifications
- Experience in social services, healthcare, or long-term care preferred
- Bachelor’s degree in Social Work, Human Services, Psychology, or related field preferred
- Knowledge of skilled nursing facility (SNF) operations is a plus
- Strong communication, organizational, and interpersonal skills
- Ability to work effectively with residents, families, and interdisciplinary teams
