Job Description
We are looking for a Physical Therapist to join our team with towns including Orland Park, Palos Park, Tinley Park, Flossmoor, Park Forest, Lockport, Homer Glen, Chicago Ridge, Calumet City, South Holland, Homewood, Chicago Heights, and parts of Grundy County

Qualifications and Responsibilities:
  • Perform in-home physical therapy evaluations and treatments
  • Develop and implement individualized rehabilitation plans of care
  • Monitor patient progress and adjust treatment plans as needed
  • Educate patients and caregivers on exercises, mobility, and safety
  • Document visits accurately and efficiently in accordance with agency guidelines
  • Communicate patient progress with the interdisciplinary care team
  • Ensure compliance with clinical, documentation, and regulatory standards
  • Supervise PTA field staff as indicated
  • Licensed Physical Therapist (PT)
  • Doctorate in Physical Therapy (DPT)
  • Home Health experience preferred, but not required
  • Strong communication and documentation skills
  • ability to work independently in a home-based setting

  • Physical ability to perform in-home visits
