    You must hold a NJ Registered Nurse license and have or be eligible to receive a NJ: School Nurse or NJ School Nurse Substitute Credential.

    FOR NEW APPLICANTS: In order to obtain a New Jersey Substitute Credential, the following information must be submitted to
    Jeannine Wright, in Human Resources
    1. Official undergraduateTranscripts* from your college or university should be sent directly to Jeannine Wright in Human Resources, 500 Tenafly Road, Tenafly, NJ 07670. (*Foreign credentials must be evaluated by an approved agency.  Please ask Human Resources for the list of approved agencies)
    2. Photocopy of Criminal History Clearance letter, OR PAYMENT RECEIPT.
    3. School Nurses Only must ALSO PROVIDE – Photocopy of New Jersey RN License, CPR, and AED current registration, and ELECTRONIC (directly from College's Clearing House)/OFFICIAL undergraduate TRANSCRIPTS  (in originally sealed envelope).                  
    NOTE:
    PLEASE CONTACT HUMAN RESOURCES AT (201) 816-4519 FOR AN APPOINTMENT TO PICK UP ADDITIONAL PAPERWORK. ALL FORMS ARE TO BE RETURNED IN PERSON. 
    (A SHORT INTERVIEW WILL BE SCHEDULED WHEN ALL PAPERWORK IS COMPLETED .)

    In order to substitute, the following additional items are required for ALL APPLICANTS:
    • Photocopy of New Jersey Substitute Credential or NJ Teacher Certification.(FOR APPLICANTS IN POSSESSION OF A TEACHING OR SUBSTITUTE CERTIFICATE)
    • A completed substitute application available on Applitrack http://www.applitrack.com/tenafly/onlineapp/ .
    • I-9 Form – Acceptable documents are noted on the form and must be presented to Jeannine Wright (photocopies are not acceptable).
    • W-4 Forms, State & Federal
    • Physical Examination
    • Mantoux Test for tuberculosis – proof of negative result.
    • New Jersey First Act Form
    • Fingerprinting (https://homeroom4.doe.state.nj.us/chr/).   (previously fingerprinted substitutes must TRANSFER their fingerprints)
    • Verification of Continuous Employment completed & signed by District where prints originated (for previously fingerprinted substitutes)
