POSITION TITLE: Nurse Care Manager

REPORTS TO (TITLE): Care Coordination Dept. Nurse Supervisor or Director of Healthcare Innovation

HOURLY RANGE: $51.39 - $64.41 HOURLY DOE

 

Job Summary: The Nurse Care Manager is responsible for direct and indirect patient care and education per their scope of practice.  S/he participates in the case management of complex clinical patients.  Evaluates and utilizes quality assurance functions for individual patients as well as through population health measures.  

Duties and Responsibilities:

  • Assists with meeting the needs of complex medical patients, including assessment and treatment, education, referrals, and follow-up care.  This may involve face-to-face, phone meetings, and review of electronic messaging and media.
  • Provides nursing care in compliance with, Santa Rosa Community Health’s policies, and State of California guidelines.  This is inclusive of assigned functions within electronic health records.
  • Visits with patients in multiple setting: Home, Hospital, SNF, clinic and community.
  • Interprets information obtained from patient and others, including the health care team, and documents all clinical care in the patient’s electronic medical record.
  • Provides general care coordination including but not limited to: patient triage, phone call-backs, prescription refills, lab ordering and interpretation, medical case management and linkage to community partners and resources. 
  • Performs education and counseling about disease management process.
  • Works closely with PCP to create and monitor a Comprehensive Care Plan.
  • Maintains tracking system.
  • Documentation on care management activities.
  • After hours call participation for case load as program indicates.
  • Maintains skills proficiency.
  • Performs related duties as requested.
