The Community Resource Navigator - 1115 Waiver Program plays a crucial role in supporting individuals as they navigate available services, helping them access life-changing resources and empowering them on their journey toward wellness and stability.

In this role, you will:

  • Completes intakes, initial assessments, referrals and education, advocacy, budgeting, screening, and provides material goods and financial assistance and documents funds disbursed as appropriate; 
  • Assess client needs and assist with negotiating systems to obtain services as needed to enhance self-sufficiency;
  • Receive and respond to phone calls, messages, and online referrals from individuals and families in need of assistance, inclusive of referrals from other community service providers;
  • Maintains a record of services, files case notes, processes payments and completes necessary documentation accurately in a professional, timely fashion; 
  • Completes data entry and provides necessary information for completion of program reports;
  • In collaboration with the designated Agency Supervisor, provides statistics and data for funding;
  • Completes 90-day client follow up to evaluate and document outcomes; 
  • Assists in the promotion of Catholic Charities through the attendance of community events, speaking opportunities, serving on local committees as requested by supervisor; 
  • Develops working relationships with other agencies, county departments, and service providers to access existing resources; 


Position Details

  • Community Resource Navigator - 1115 Waiver Program Salary: $20.00/hour 
  • Schedule: Monday–Friday 12pm-4pm, 20 hours/week
  • No subject-to-call shifts required


Qualifications

  • Education:
    • High School Diploma or GED
  • Experience:
    • 1–3 years’ experience with relevant experience in Human Services or a related field
  • Skills:
    • Microsoft Office
      • Email
    • Using databases
    • Ability to maintain strict confidentiality
  • Other requirements:
    • Valid Driver's License
    • Proof of current car insurance and registration and access to a reliable, working vehicle
