Job Description

Job Title: Nurse Liaison (LPN / Medical Assistant)

About the Role: Are you an organized, self-motivated healthcare professional looking for an autonomous and impactful role? Spectrum Healthcare Solutions is seeking a dedicated Nurse Liaison to coordinate services across long-term care, skilled nursing, inpatient rehab, and assisted living facilities. Your primary goal will be to bridge the gap between our medical providers and facility staff to improve overall patient outcomes and decrease hospital readmissions.

This role is highly autonomous, requires a strong work ethic, and includes a significant administrative component. If you have a background as a Medical Assistant or LPN and excel in relationship-building and clinical organization, we want you on our team!

Key Responsibilities:

  • Manage the daily routine of physician rounding within assigned facilities.

  • Arrive two hours prior to the physician's scheduled rounding time to prepare equipment, assess clinical situations, and gather all necessary information for patient evaluations.

  • Assist the physician directly during patient examinations, take orders, and enter them into the Electronic Medical Record (EMR).

  • Reconcile medications, educate patients about their treatments, and report lab and imaging findings to the physician during rounds.

  • Accurately transcribe and prepare all physician notes in the EMR within 24 hours of the patient visit.

  • Act as the primary liaison between facility staff, medical providers, and patients to promote clear communication.

  • Communicate daily with assigned Nurse Practitioners regarding required visits and patient needs, and follow up with facility staff to ensure all new orders are implemented.
