Greenfield Rehabilitation Agency is seeking a PRN Physical Therapist (PT) to join our growing therapy team at our senior living community in New Richmond, WI and its surrounding communities.  This is a great opportunity for a dedicated and flexible PT looking to make a meaningful impact, with the potential to grow into a consistent part-time role.

Your Responsibilities

  • Evaluate and treat patients in a skilled nursing setting

  • Develop individualized care plans to improve mobility, strength, and function

  • Collaborate with the interdisciplinary team to ensure continuity of care

  • Document patient progress in accordance with regulatory and company standards

Qualifications


  • Graduate of an accredited school with a bachelors, master’s, or doctoral degree in Physical Therapy.
  • License as required by the State of Wisconsin to practice as a Physical Therapist.
