Schedule: Full‑time | Monday–Friday, 8:00 AM–4:30 PM EST

Salary Range:  
MA $16.00 to $19.00 Hourly
LPN $20.00 to $25.00 Hourly

We are seeking experienced, patient‑focused Chronic Pain Care Navigators who bring both clinical knowledge and compassion to help patients manage pain more effectively and improve quality of life.

This role is ideal for Medical Assistants or LPNs with a strong background in pain management, who value proactive outreach, patient education, and continuity of care. Our patients depend on regular touchpoints and trusted relationships—this is not a passive or appointment‑only role.


⭐ Why This Role Matters

As a Chronic Pain Care Navigator, you become a consistent point of contact for patients managing long‑term pain conditions. You help patients understand treatment plans, navigate pain management options, address barriers to care, and remain engaged with their providers.

More than 75% of your day is spent directly communicating with patients, primarily through outbound calls. Your role is to drive engagement, reinforce care plans, and support patients as they work toward improved function and wellbeing.


💼 What You’ll Do

  • Make up to 20 outbound patient calls daily
  • Conduct patient assessments within scope related to chronic pain history, symptoms, and treatment adherence
  • Support care coordination for pain management services, therapies, and follow‑up appointments
  • Reinforce provider‑directed care plans, including medication education and non‑pharmacologic strategies
  • Educate patients on pain management options, expectations, and self‑management techniques
  • Identify barriers to care and escalate concerns to the appropriate clinical team members
  • Serve as a communication bridge between patients, providers, clinics, and care teams
  • Track patient progress and engagement over time
  • Document all patient interactions accurately across multiple healthcare systems and tools

🧠 What You Bring

Required

  • Medical Assistant (MA) or Licensed Practical Nurse (LPN)
  • Minimum of 3 years of pain management experience
  • Strong understanding of chronic pain conditions and treatment approaches
  • Experience supporting patients with long‑term or complex conditions
  • Excellent communication and patient engagement skills
  • A HIPAA‑compliant home office with reliable high‑speed internet
  • Ability to work a consistent Monday–Friday schedule without rotating hours

Preferred

  • Experience in care navigation, care coordination, or case management
  • Background in multidisciplinary pain management settings
  • Comfort working with EHRs and healthcare software platforms
  • Experience working with high‑needs or chronic condition populations
