🩹 Position Summary



We’re seeking compassionate Licensed Practical Nurses (LPNs) to join our growing Dublin, Ohio team.

As an LPN in home health, you’ll provide direct patient care, monitor progress, and ensure the highest quality outcomes — all while working independently with the support of our experienced clinical leadership team.







🧾 Key Responsibilities



Obtain and document patient medical history and current condition

Conduct brief physical assessments (vitals, ambulatory status, mental status, etc.)

Implement physician-ordered care plans and nursing interventions

Educate patients and families about care routines and health management

Provide skilled nursing care and maintain confidentiality per HIPAA regulations

Evaluate the effectiveness of care and update the plan of care as needed

Submit all documentation and visit notes in a timely manner

Collaborate with the interdisciplinary care team to ensure excellent outcomes
